Simone Biles walked the red carpet at the 2026 ESPYs on July 15, showcasing her recent physical transformation.
She stunned in a strapless metallic gown with a corseted sweetheart neckline and a high-low hem.
According to OK Magazine, Biles attended the event with her husband, Indianapolis Colts safety Jonathan Owens. She accessorized the look with a diamond necklace, earrings, and a ring. Her hair featured a side part with waves.
This appearance comes about eight months after Biles shared in a TikTok video that she had undergone three plastic surgeries.
“I’ve had three plastic surgeries, and two of them you would never be able to tell,” Biles said in the video posted on October 29, 2025. She then egged on her followers to guess which procedures she had.
Biles revealed this information during a “get ready with me” video, and in it, she listed facts about herself while talking to Owens on speakerphone.
Three weeks later, on November 19, 2025, Biles posted another TikTok video explaining one of the procedures: a breast augmentation performed on June 16, 2025, by Dr. Kriti Mohan, a board-certified plastic surgeon in Houston.
Biles said her implants were “310 high-profile, extra-filled, silicone” and placed underneath her chest muscle for a more natural look.
She added that her recovery was harder than she expected, stating that friends had told her beforehand that the pain would be manageable.
Biles’ admissions came after weeks of online speculation about whether she had undergone cosmetic surgery.
In July 2025, fans and a plastic-surgery-focused social media account speculated about a breast augmentation after Biles posted vacation photos from Belize.
Dana Omari, who runs the Instagram account @IGFamousByDana, shared one of Biles’ photos and commented that “Simone Biles with the breast aug!! She looks incredible.”
Biles has also addressed a separate cosmetic procedure. In a TikTok video for her 27th birthday, she mentioned receiving Botox injections in her forehead but chose not to continue with the treatment. She noted that the injections caused her eyebrow to move unexpectedly during gymnastics practice.
Biles attended the 2025 ESPYs, marking her first appearance at the awards show since 2017. She was nominated for Best Athlete in Women’s Sports and Best Championship Performance for her all-around gold medal at the 2024 Paris Olympics.
Biles wore a custom gown by designer Zac Posen, created in partnership with Athleta. The gown was made from the fabric of the brand’s Transcend Open Back Tee and hand-stitched with bugle beads by over 25 artisans, taking more than 1,000 hours to complete.
“I wanted to create something that speaks to her power, her movement, and her identity,” Posen said. Biles also took this opportunity to support her longtime friend Serena Williams, who was inducted into the U.S. Olympic & Paralympic Hall of Fame just days earlier.
Biles and Owens have been married since 2023 after dating for about three years.
