A 20-year-old man fatally shot by Irvine police during a mental health crisis has been identified as Daniel Alfons Meltvedt of Irvine, authorities announced Wednesday.

Meltvedt called the Irvine Police Department on the afternoon of July 21 and said he was armed with a knife, wanted to harm himself and might harm others, according to an updated account from the department.

Police said Meltvedt then left his home carrying a knife and walked onto Sungate near Silkwood.

Members of the department’s Crisis Negotiation Team responded and spoke with Meltvedt for more than an hour, repeatedly urging him to put down the knife and surrender so he could receive care.

Police said Meltvedt eventually charged toward an officer with the knife raised, prompting the officer to open fire.

Officers provided medical aid before Meltvedt was taken to a hospital, where he died.

The department said officers had previously responded to several mental health-related incidents involving Meltvedt and connected him with mental health resources after each encounter.

I-CARE personnel and Orange County Fire Authority crews were standing by during Tuesday’s negotiations to provide medical and mental health assistance.

The shooting remains under investigation by Irvine police and the Orange County District Attorney’s Office.

Police said they expect to release a critical incident video containing 911 calls and footage from officers’ body-worn cameras in the coming weeks.
