A 69-year-old delivery-truck driver developed deep wrinkles, thickened skin, and other signs of severe sun damage almost entirely on the left side of his face. He had spent 28 years behind the wheel, placing that side closest to the driver-side window for much of his working life.

The unusual pattern was documented in a clinical case published in the New England Journal of Medicine. Doctors diagnosed unilateral dermatoheliosis, a form of long-term skin damage caused by uneven exposure to ultraviolet radiation. The case showed that sunlight passing through a closed vehicle window can affect the skin gradually, even without frequent sunburn.

The man had experienced what the medical report described as “gradual thickening and wrinkling” on the left side of his face for about 25 years. The changes caused no symptoms, but the contrast between the two sides became increasingly visible as the damage accumulated.

UVA Exposure Caused Uneven Facial Aging

Doctors found pronounced wrinkles, thickened skin, open comedones, and small nodules on the more exposed side of the man’s face. A tissue examination also revealed changes in the dermis, the supportive layer beneath the skin’s surface, including damaged elastic material and small cysts around fine hair follicles.

The findings were consistent with Favre-Racouchot syndrome, a condition associated with heavily sun-damaged skin. It commonly includes coarse wrinkles, yellowish thickening, blackheads, and changes around hair follicles. In this case, those features appeared mainly on the side facing the truck window.

A CBS News report on the medical case noted that the driver received most of the exposure on his left side because of the many hours he spent on the road. His right side, which remained farther from the window, showed much less visible damage.

The contrast represents severe facial photoaging, not a difference in the man’s biological age. Both sides aged at the same rate, but one side absorbed far more ultraviolet radiation over decades of driving.

UVA Rays Can Pass Through Closed Windows

Sunlight contains several types of ultraviolet radiation. UVB radiation acts mainly on the skin’s surface and is closely associated with redness and sunburn. Ordinary window glass blocks much of UVB, which can make sitting inside a vehicle feel protected from the sun.

UVA radiation behaves differently. It can pass through window glass before penetrating the epidermis and the upper layers of the dermis. As a result, a driver may receive repeated UVA exposure on the face, neck, hands, and arms without feeling the immediate burning sensation commonly associated with ultraviolet damage.

Over many years, UVA can thicken the epidermis and the stratum corneum, the outermost layer of dead cells that protects the body. It can also damage elastic fibers in the skin, which help tissue stretch and return to its original shape. As those fibers deteriorate, the skin becomes rougher, less flexible, and more deeply wrinkled.

The driver’s face offered a particularly clear comparison because age, genetics, diet, and most other personal factors were shared by both sides. The major difference was the amount of sunlight each side received, creating a visible record of long-term occupational exposure.

Long-term UV Damage Extends Beyond Wrinkles

The case focused on visible skin aging, but ultraviolet exposure can also damage cells beneath the surface. UVA can contribute to DNA damage and cellular injury, while UVB is strongly linked to mutations involved in the development of skin cancer.

The medical report did not identify skin cancer in the driver. However, doctors recommended regular monitoring because of the extent and duration of his sun exposure. They also advised sun protection and topical retinoids, vitamin A-derived treatments that can improve some signs of damaged and thickened skin.

The case does not mean every person who drives regularly will develop such an extreme difference between the two sides of the face. It does show how small, repeated exposures can accumulate across decades, particularly for truck drivers, delivery workers, taxi drivers, and others who spend much of the day beside a vehicle window.

Protection needs to address both forms of ultraviolet radiation. Broad-spectrum sunscreen is designed to cover UVA and UVB rays, while clothing, hats, shade, and suitable ultraviolet-filtering window treatments can reduce the amount of radiation reaching exposed skin.

The Case Shows How Occupational Sun Exposure Builds Over Time

The photograph gained attention because the contrast was immediately visible, but the medical importance lies in the process behind it. The changes did not appear after a single intense exposure. They developed slowly as the same area of skin received sunlight through the window during years of routine driving.

That pattern makes the case relevant to occupational sun exposure, including work performed inside vehicles. A person does not need to work outdoors with an uncovered head to receive a substantial cumulative dose of ultraviolet radiation.

The original clinical report in the New England Journal of Medicine connected the one-sided damage to the driver’s long history on the road and the ability of UVA to penetrate glass. The condition provided a clear visual example of how everyday exposure can alter the structure and appearance of skin over time.
